If you are new to our practice you will need to complete our Pre-Treatment

form so we can give you individualized care..

You can print the form, fill it in at your leisure and bring in with you to your first appointment.

Alternatively, please arrive 10 minutes prior to your appointment time to fill it in here at the practice.
